Community Follow up Worker

Overview

The Follow-Up Worker functions as part of a Case Management Team, assisting the Case Manager and Case Manager Tech in the provision of services to individuals and families living with HIV/AIDS.

Responsibilities

1. Liaison between Case Manager and Clients. Advocating, scheduling fair hearings, escorting clients to hospital, doing home visits, etc. 2. Complete client pre-screenings and Medicaid eligibility. 3. Administrative duties consist of a variety or paperwork. 4. Responsible for maintenance of the case record. 5. Participate in staff meeting/training. 6. Participate in advocacy and direct action to end the twin crisis of AIDS and homelessness.

Qualifications

1. High school diploma or equivalent diploma. 2. Ability to connect with and engage the target client population. 3. Sensitivity to HIV/AIDS, substance use, mental illness and other related issues. 4. Ability to maintain case records and filing systems.
