Overview

MSW/ LMSW with at least two years of clinical experience in providing health care and psychosocial support for enrolled clients.

Responsibilities

The candidate will be responsible to perform psychosocial assessment, reassessment, treatment plan, case management services, crisis intervention, individual and group counseling. Candidate should possess skills in working with substance use, mental illness and HIV homeless population.

Qualifications

Prior experience in an adult day health care setting is preferred. Excellent communication skills and organizational skills are required. Proficiency in Spanish is also preferred.
