Overview

Service coordination for clients. Maintain client records, assist clients with entitlements, provide ongoing psycho-social support and referrals for assistance with independent living skills, health and legal services.

Responsibilities

Supervise job trainees and assist in the development of career goals and assist in the implementation of a plan to move towards such goals.

Qualifications

Ability to work in a team management system • Bachelor’s Degree. • Two years relevant experience. Master’s Degree may be substituted for experience. • Good verbal and written communication skills. • Good interpersonal skills and ability to work in a team. • Strong supervisory skills. • Knowledge of HIV/AIDS, substance use, homelessness and mental illness. • Knowledge of City, State, and Federal entitlement systems.
