Overview

The Substance Abuse Counselor – CASAC is responsible for providing multiple aspects of biopsychosocial care for a client caseload, including screening, orientation, assessment, case management, crisis intervention, indi¬vidual and group counseling, discharge planning, and overall care coordination. The Substance Abuse Counselor – CASAC has primary responsibility for coordination, development, supervision and service provision of substance use counseling services, including completion of all substance use assessments, reassessment and treatment planning.

Responsibilities

1. Provides direct individual and group counseling and treatment planning / care coordination to population. 2. Completing initial psychosocial assessments and treatment plan for all clients within four visits to the program, in a period not to exceed thirty (30) days. 3. Completes initial substance use assessments for all clients within four visits to the program, in a period not to exceed thirty days. Reassesses client every ninety (90) days, and address progress in treatment plan making appropriate revisions to treatment plan as necessary. 4. Participates in interdisciplinary care planning and daily rounds. 5. Provides individual and group mental health, substance use counseling. Facilitates or supervises recovery, relapse prevention, and other support groups. 6. Provides case management services to clients to assist with entitlement and other biopsychoso¬cial needs, as indicated in their assessment / reassessment. 7. Confirms that all case management services and appointments are arranged for the client in a timely manner. 8. Monitors aforementioned services on an ongoing basis to ensure that newly identified needs of clients are addressed expeditiously. 9. Maintains client caseload for the purpose of coordinating care both within the program and with outside care case management providers. 10. Assists with the development of linkages with needle exchange, detoxification, rehabilita¬tion, and methadone maintenance programs to facilitate client referrals. 11. Serves as a consultant to interdisciplinary team on substance use issues and their impact on social functioning. 12. Contributes toward the formulation of care plan goals and objectives for all clients based on comprehensive assessments. 13. Reports significant observations and interactions with clients via daily rounds, service logs, and chart notes. 14. Performs other duties as directed by Program Director. 15. Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS. Reports to: Director of Mental Health or appropriate designee

Qualifications

Completion of CASAC certification is required. One to two years of Bachelor’s level credit or experience equivalent to education is desirable. Experience in providing counseling services to persons with HIV/AIDS is required. Experience in an adult day health care / chemical dependency program is required. Experience with group facilitation required.
